Rare and Unusual Complications of Twin Pregnancies

Monozygotic twinning occurs when a single fertilized ovum splits after conception. The outcome depends on the timing of division. In the first 72 hours after fertilization, a diamniotic, dichorionic twin develops; between days 4 and 8, a diamniotic, monochorionic twin develops; approximately 8 days after fertilization, a monoamniotic, monochorionic twin develops; and after day 12, conjoined twins (shown) develop. Monozygotic twins occur in 4 in 1,000 births. Diagnosis is best done by ultrasound in the first or early second trimester.

                                                                Cojoined  Twins

  • Conjoined twins are a rare event in the world's delivery rooms.
  • They occur about once in every 50,000 births, but 40% are stillborn, and, curiously, 70% are female.
  • Conjoined twins are always identical: the product of a single egg that for some unknown reason failed to divide fully into separate twins during the first three weeks of gestation.

Rare  Cojoined Twins

Manar was born with a rare condition known as craniopagus parasiticus, which occurs when an embryo begins to split into identical twins but fails to complete the process. One of the conjoined twins fails to develop fully in the womb.

As in the case of a girl who died after similar surgery in the Dominican Republic a year ago, the second twin had developed no body. The head that was removed from Manar had been capable of smiling and blinking but not independent life, doctors said.

The sisters are believed to suffer from a condition known as dicephalic parapagus – an extremely unusual form of twin conjoinment where only a single body develops.
Because they share the same body, it is not possible to separate dicephalic parapagus conjoined twins.
The birth of dicephalic parapagus conjoined twins – who develop after a fertilized egg cell fails to divide fully – is extremely rare, with most cases occurring in southwest Asia and Africa.

“Sirenomelia, alternatively known as Mermaid Syndrome, is a very rare congenital deformity in which the legs are fused together, giving them the appearance of a mermaid’s tail.
This condition is found in approximately one out of every 100,000 live births[1] (about as rare as conjoined twins) and is usually fatal within a day or two of birth because of complications associated with abnormal kidney and urinary bladder development and function.”

Other fertility treatments, like IVF can also result in twin or higher order multiple pregnancies.
